Summary Care Records

Summary Care Records is an electronic record which contains important information about any medicines you may take, allergies you suffer from and any bad reactions to medication you may have had. This record is available to healthcare staff when you are seen in an emergency or when the surgery is closed.  Having this information can prevent mistakes being made when you are being treated.

It is not compulsory to be part of this scheme.  If you do not want a Summary Care Record let the surgery know by filling in an opt-out form available at the surgery.
