New Patient Registration

We will ask you to complete a registration form available from reception and request you make an appointment with one of our nurses for a health check and to obtain medical history before your medical notes arrive from your previous GP. 

Please ensure you bring list of any current medication. 
 

You will be eligible to register with us if you live within 5 miles (8km) of the surgery.

We are proud to be a Safe Surgery for everyone in our community.  Everyone in the UK has the right to free care from a GP  If you do not have proof of ID or an address you can still register at the practice.  Reception will not ask for immigration status and will not share your information with the Home OFfice unless a serious crime is involved.
